INTRODUCTION: Currently, there is a lack of prospective studies to unify criteria about type and time for postoperative immobilisation in surgical distal radius fractures. The aim of this study is to compare functional and radiological results in two groups of distal radius fractures treated with internal fixation with locking plate, and immobilised with antebrachial splint or compression bandage for 3 weeks. MATERIAL AND METHOD: A randomised clinical trial was carried out with two parallel groups with 3, 6, and 12 weeks of follow-up. Main and secondary functional variables were measured, such as pain on VAS scale, values on PRWE, DASH and MRS scale, range of motion in flexion-extension, complications, etc. In addition, some radiological variables were measured at preoperative period and one week after surgery, such as union time, dorsal displacement, shortening, ulnar variance, etc. RESULTS: A total of 62 patients were evaluated: 27 immobilised with bandage and 35 with splint. Analysis of the results obtained showed significant differences in both groups for almost all radiological variables from pre to postoperative period, and for all functional variables from 3 to 12 weeks after surgery. No significant differences were found between the two groups for any of the radiological and functional variables evaluated (VAS 3-12 weeks: p=.584; PWRE 3-12 weeks: p=.248; flexion range of motion 3-12 weeks: p=.959; extension range of motion: p=.50; union time: p=.89). CONCLUSIONS: We do not find clinical or radiological differences between immobilisation with antebrachial splint or compression bandage for distal radius fractures operated with locking plate. A greater number of patients and follow-up are necessary to extrapolate the results to the general population and to establish criteria for good postoperative management of these fractures.

Objetivo principal: Conocer la epidemiologia de las fracturas del cuarto distal del radio en el esqueleto en crecimiento y el tratamiento realizado en el CHPR en los años 2017 y 2018. Objetivos específicos: Valorar re-desplazamiento, necesidad de re manipulación, complicaciones, re-fractura. Metodología: Estudio observacional descriptivo retrospectivo. Criterios de inclusión: pacientes de 0 a 14 años con fractura de radio distal (fisaria, metafisaria y suprametafisaria) valorados en el CHPR entre 1 enero del 2017 y 31 diciembre del 2018. Criterios de exclusión: pacientes con radiografía normal, fracturas en miembros con malformaciones, infecciones óseas, patología tumoral maligna o benigna. Obtención de datos: Valoración de radiografías de puño y antebrazo realizadas en el CHPR en 2017 y 18, iniciales y evolutivas. Se analizaron: edad, sexo, fecha fractura, topografía de la fractura, desplazamiento inicial, tratamiento, evolución radiográfica y complicaciones. Resultados: se incluyeron 662 pacientes. Siendo en su mayoría de género masculino (65%), con una media de 9 años, miembro derecho (61%), en los meses de verano (36%). En cuanto a la topografía se evidenció una frecuencia mayor en fracturas tipo rodete y metafisarias (31.72% y 31.57%), seguido por suprametafisaria (18.43%) y fisaria (18.28%). El tratamiento realizado fue ortopédico en el 86.56% de los casos, mientras que quirúrgico fue el 12.84%, en su gran mayoría con alambres de Kirschner (11,2%). El tiempo de inmovilización promedio fue de 6 semanas, con un porcentaje de complicaciones del 14.05% del total de las fracturas. Conclusiones: Se valoraron las características de los pacientes y fracturas de radio distal en el CHPR en los años 2017 y 18, siendo un total de 662 fracturas, en las que su mayoría se realizó tratamiento ortopédico con un índice de complicaciones que ronda el 14%, siendo mayor cuanto mayor es el desplazamiento inicial de la fractura.

Em relação à topografia, foi evidenciada maior frequência nas fraturas da borda e metafisárias (31,72% e 31,57%), seguidas das suprametafisárias (18.43%) e fisárias (18.28%). O tratamento realizado foi ortopédico em 86.56% dos casos, enquanto cirúrgico em 12.84%, sendo a maioria com fios de Kirschner (11,2%). O tempo médio de imobilização foi de 6 semanas, com percentual de complicações de 14.05%. Conclusões: Foram avaliadas as características dos pacientes e fraturas do rádio distal no CHPR nos anos de 2017 e 18, com um total de 662 fraturas (0,9 por dia), em que a maioria foi submetida a tratamento ortopédico com índice de complicações que fica em torno de 14%, sendo maior quanto maior for o deslocamento inicial da fratura.


Title: Fractures of the distal end of the radius in the immature skeleton. Epidemiological study at the Pereira Rossell Hospital Center. Main objective: To know the epidemiology of fractures of the distal fourth of the radius in the growing skeleton and the treatment carried out in the CHPR in the years 2017 and 2018. Specific objectives: To assess re-displacement, need for re-manipulation, complications, re- fracture. Methodology: Retrospective descriptive observational study. Inclusion criteria: patients aged 0 to 14 years with distal radius fracture (physeal, metaphyseal and suprametaphyseal) evaluated at the CHPR between January 1, 2017 and December 31, 2018. Exclusion criteria: patients with normal radiographs, fractures in limbs with malformations, bone infections, malignant or benign tumor pathology. Data collection: Assessment of fist and forearm X-rays performed at the CHPR in 2017 and 18, initial and evolutionary. The following were analyzed: age, sex, fracture date, fracture topography, initial displacement, treatment, radiographic evolution and complications. Results: 662 patients were included. Being mostly male (65%), with an average of 9 years, right limb (61%), in the summer months (36%). Regarding the topography, a higher frequency was evidenced in rim and metaphyseal fractures (31.72% and 31.57%), followed by suprametaphyseal (18.43%) and physeal (18.28%). The treatment performed was orthopedic in 86.56% of the cases, while surgical was 12.84%, mostly with Kirschner wires (11.2%). The average immobilization time was 6 weeks, with a percentage of complications of 14.05%. Conclusions: The characteristics of the patients and fractures of the distal radius in the CHPR in the years 2017 and 18 were evaluated, with a total of 662 fractures (0.9 per day), in which the majority underwent orthopedic treatment with an index of complications that is around 14%, being greater the greater the initial displacement of the fracture.

Antecedentes y objetivo: Dos de cada tres fracturas de muñeca están desplazadas pero la mayoría pueden tratarse de forma conservadora tras una reducción cerrada. El dolor que refieren los pacientes durante la reducción cerrada de estas fracturas es variable y el mejor método para controlarlo no está bien establecido. El objetivo de este estudio fue evaluar el dolor durante la reducción cerrada de fracturas de radio distal tras utilizar el bloqueo del hematoma como método anestésico. Material y método: Estudio clínico transversal que incluyó a todos los pacientes que presentaron una fractura aguda de radio distal que requirió reducción cerrada e inmovilización durante un periodo de seis meses en dos Hospitales Universitarios. Se registraron datos demográficos, clasificación de la fractura, dolor percibido mediante una escala visual analógica en diferentes momentos de la reducción y complicaciones. Resultados: Se incluyeron 94 pacientes consecutivos. La edad media fue de 61 años. La puntuación media del dolor en la evaluación inicial fue de seis puntos. Tras realizar el bloqueo del hematoma, el dolor percibido durante la maniobra de reducción descendió a 5,1 puntos en la muñeca, pero aumentó a 7,3 puntos en los dedos. El dolor disminuyó a 4,9 puntos durante la colocación del yeso y alcanzó 1,4 puntos tras la colocación del cabestrillo. El dolor referido fue mayor en las mujeres en todos los momentos. No hubo diferencias significativas según el tipo de fractura. No se observaron complicaciones neurológicas ni cutáneas. Conclusiones: El bloqueo del hematoma es un método solo ligeramente efectivo para reducir el dolor en la muñeca durante la reducción cerrada de las fracturas de muñeca. Esta técnica disminuye ligeramente el dolor percibido en la muñeca y no reduce el dolor en los dedos. Otros métodos de reducción u otras técnicas analgésicas pueden ser opciones más efectivas.(AU)

Demographic data, fracture classification, perceived pain using a visual analog scale at different times of reduction and complications were registered. Results: Ninety-four consecutive patients were included. Mean age was 61 years. Mean pain score at initial assessment was 6 points. After the hematoma block, the perceived pain during the reduction manoeuvre improved to 5.1 points at the wrist, but increased to 7.3 points at the fingers. Pain decreased to 4.9 points during cast placement and reached 1.4 point after sling placement. The reported pain was higher in women at all times. There were no significant differences according to the according to the type of fracture. No neurological or skin complications were observed. Conclusions: The hematoma block is only a mild effective method to reduce the wrist pain during closed reduction of distal radius fractures. This technique decreases slightly the perceived pain in the wrist and does not reduce the pain in the fingers. OBJECTIVES: We evaluated the utility of 3D printing technology for preoperative planning in the treatment of intra-articular fractures of the distal radius in relation to the improvement of surgical technique, radiological and clinical results. MATERIAL AND METHODS: A total of 30 patients with 2B and C fractures of the AO classification were operated on by a single surgeon with a volar plate, randomly divided into two groups, 15 of them with conventional planning (Rx and CT) and 15 adding a 3D model of the fracture and the previous simulation of the intervention. Simulation time, surgical time in minutes, radioscopy time in minutes, loss of material expressed in lost screws were recorded. Clinical evaluation based PRWE questionnaire and full radiographic analysis was done for all patients with a mean follow-up of 6 months by an independent, blinded observed. RESULTS: No statistically significant differences were observed in the PRWE questionnaire (p=0.22), nor were we observed differences in the radiological values, except in relation to the articular step (p=0.028), which represents statistical significance, but in both groups the median was of 0.0 (0.0-0.0). We also did not see statistically significant differences in surgical times (p=0.745), radioscopy (p=0.819) or in the loss of synthesis material (p=0.779). CONCLUSIONS: 3D printing has not improved the parameters studied in relation to routinely operated patients.

The purpose of this study was to assess pain during closed reduction of distal radius fractures after using haematoma block as method of anaesthesia. PATIENTS AND METHODS: Cross-sectional clinical study including all patients who presented an acute fracture of the distal radius requiring closed reduction and immobilisation during a six-month period in two University Hospitals. Demographic data, fracture classification, perceived pain using a visual analogue scale at different times of reduction and complications were registered. RESULTS: Ninety-four consecutive patients were included. Mean age was 61 years. Mean pain score at initial assessment was 6 points. After the haematoma block, the perceived pain during the reduction manoeuvre improved to 5.1 points at the wrist, but increased to 7.3 points at the fingers. Pain decreased to 4.9 points during cast placement and reached 1.4 point after sling placement. The reported pain was higher in women at all times. There were no significant differences according to the according to the type of fracture. No neurological or skin complications were observed. SUMMARY: Distal radius fractures are one of the most common orthopedic injuries encountered by orthopedic surgeons. Correction of dorsal tilt and radial height is essential to restore normal biomechanics of the wrist joint. Comprehensive knowledge of the morphometry of the distal radius of the local population becomes critical for the treating surgeon. This study aims to report the morphometry of the distal radius in the Anatolian population and compare it with similar studies in other races and humans. Radiographs of one hundred and twenty-four people were included in the study. Four radiological parameters were examined on all radiographs: radial height, radial tilt, ulnar variance, and palmar tilt. Radial tilt was 23.35°±1.96; palmar tilt was 15.7°±2,87 radial height (mm) was 10.55±4.34, ulnar variance (mm) was 0.32±1.79. The highest rate of negative ulnar variance was found. According to the study's results, reference data varying by race for anatomical fit should be considered in treating DER injuries.


Las fracturas de la parte distal del radio son probablemente las lesiones ortopédicas más comunes que encuentran los cirujanos ortopédicos. La corrección de la inclinación dorsal y la altura radial es esencial para restaurar la biomecánica normal de la articulación de la muñeca. El conocimiento integral de la morfometría del radio distal de la población local es importante para el cirujano tratante. Este estudio tuvo como objetivo reportar la morfometría de la parte rdistal del radio en la población de Anatolia y compararla con estudios similares en otras razas y humanos. Se incluyeron ciento veinticuatro radiografías simples consecutivas de la articulación de la muñeca. Se examinaron cuatro parámetros radiológicos: altura radial, inclinación radial, variación ulnar e inclinación palmar. El ángulo de inclinación radial fue de 23,35±1,96; el ángulo de inclinación palmar fue de 15,7±, la altura radial (mm) fue de 10,55±4,34, la varianza ulnar (mm) fue de 0,32±1,79. Se encontró la tasa más alta de varianza ulnar negativa (43,5%). Los resultados de este estudio deben tenerse en consideración al tratar fracturas de la parte distal del radio, con datos de referencia que varían según la raza para el ajuste anatómico.

Resumen: Introducción: las fracturas del radio distal son las más comunes en extremidades superiores. Estandarizar las mediciones radiográficas para su abordaje quirúrgico es importante. Este estudio midió la reproducibilidad intra/interobservador de parámetros radiográficos asociados al éxito quirúrgico en estas fracturas. Material y métodos: diseño transversal retrospectivo con datos secundarios de expedientes clínicos. Se evaluaron radiografías de 112 fracturas en planos posteroanterior y lateral por dos traumatólogos estandarizados en toma de mediciones para calcular cinco parámetros indicativos de éxito postquirúrgico: altura radial, inclinación radial, inclinación volar, varianza cubital y escalón articular. La reproducibilidad de distancias y ángulos se evaluó con el método de Bland-Altman calculando: diferencia media entre mediciones, rango a ± 2 DE y proporción de mediciones fuera de ± 2 DE. El éxito postquirúrgico se comparó en pacientes con/sin obesidad según la media de las dos mediciones de cada evaluador. Resultados: el evaluador 1 tuvo la mayor diferencia intraobservador en altura radial (0.16 mm) y la mayor proporción fuera de ± 2 DE en varianza cubital (8.1%); el evaluador 2 tuvo la mayor diferencia en inclinación volar (1.92o) y la mayor proporción en inclinación radial (10.7%). La mayor diferencia interobservador fue en varianza cubital (1.02 mm) y la mayor proporción fuera de ± 2 DE en altura radial (5.4%). La inclinación radial tuvo la mayor diferencia (1.41o) con 4.5% de mediciones fuera de ± 2 DE. La varianza cubital y la inclinación volar tuvieron la mayor diferencia de éxito postquirúrgico entre evaluadores, sobre todo en pacientes con obesidad. Conclusión: mejorar la calidad radiográfica y estandarizar las mediciones resulta en indicadores más reproducibles.


Abstract: Introduction: distal radius fractures are the most common in upper extremities. Therefore, it is important to standardize radiographic measures for their surgical approach. This study assessed the intra/interobserver reproducibility of radiographic parameters associated with surgical success of distal radius fractures. Material and methods: retrospective cross-sectional design of secondary data extracted from clinical records. Posteroanterior and lateral X-rays of 112 distal radius fractures were assessed by two trauma specialists standardized in the measurements required to compute five parameters indicative of postoperative success: radial height, radial inclination, volar tilt, ulnar variance, and articular stepoff. The reproducibility of distances and angles was evaluated using the Bland-Altman method, calculating the mean of the difference between measurements, the range at ± 2 SD, and the proportion of measurements outside ± 2 SD. Postoperative success was also compared between patients with and without obesity according to the mean of the two measures made by each evaluator. Results: evaluator 1 had the largest intra-observer difference in radial height (0.16 mm) and the largest proportion outside ± 2 SD in ulnar variance (8.1%); evaluator 2 had the largest difference in volar tilt (1.92o) and the highest proportion in radial inclination (10.7%). The largest inter-observer difference was for ulnar variance (1.02 mm) and the largest proportion outside ± 2 SD. for radial height (5.4%). Radial tilt had the largest difference (1.41o) with 4.5% of measurements outside ± 2 SD. Ulnar variance and volar tilt had the largest difference in postoperative success between evaluators, especially in patients with obesity. Conclusion: improving the radiographic quality and standardizing the measurements results in more reproducible indicators.

Resumen: Introducción: El tumor de células gigantes (TCG) es un tumor óseo intramedular benigno que surge con frecuencia en los extremos de los huesos largos. Después del fémur distal y la tibia proximal, el radio distal es el tercer sitio más afectado con tumores, particularmente agresivos. Nuestro objetivo es la presentación del caso clínico de una paciente con diagnóstico de TCG de radio distal clasificada en grado III de Campanacci que recibió un tratamiento ajustado a sus posibilidades económicas. Reporte de caso: Paciente femenino de 47 años, sin solvencia económica y sin ningún servicio médico. El tratamiento incluyó resección en bloque, reconstrucción con autoinjerto de peroné distal y artrodesis radiocarpiana con placa de compresión bloqueada. Dieciocho meses después, la paciente presentaba una buena fuerza de prensión (80% respecto al lado sano) y tenía una función motora fina en la mano. La muñeca presentó estabilidad con pronación de 85o, supinación de 80o, flexión-extensión de 0o y una puntuación de 6.7 en el cuestionario de evaluación de resultados funcionales DASH. Su evolución radiológica a cinco años después de su cirugía continuó sin datos de recidiva local y afectación pulmonar. Conclusión: El resultado en esta paciente, junto con los datos publicados, indican que la técnica de resección tumoral en bloque, más el autoinjerto de peroné distal y la artrodesis con placa de compresión bloqueada proporcionan un resultado óptimo de funcionalidad para el tumor radial distal grado III a bajo costo.


Abstract: Introduction: Giant cell tumor (GCT) is a benign intramedullary bone tumor that frequently arises at the ends of long bones. After the distal femur and proximal tibia, the distal radius is the third most affected site with particularly aggressive tumors. Our objective is the presentation of the clinical case of a patient diagnosed with distal radius GCT classified in grade III of Campanacci who received a treatment adjusted to her economic possibilities. Case report: A 47-year-old female, without economic solvency and with some medical service. Treatment included block resection, reconstruction with distal fibula autograft, and radiocarpal fusion with blocked compression plate. Eighteen months later, the patient had good grip strength (80% on the healthy side) and had fine motor function in the hand. The wrist presented stability with pronation of 85o, supination of 80o, flexion-extension of 0o and a score of 6.7 in the DASH functional outcomes assessment questionnaire. His radiological evaluation five years after his surgery continued with no evidence of local recurrence and pulmonary involvement. Conclusion: The result in this patient, together with the published data, indicate that the block tumor resection technique, plus distal fibula autograft and arthrodesis with blocked compression plate provide an optimal result of functionality for the grade III distal radial tumor at low cost.

RESUMEN Introducción: la fractura del radio distal es una de las entidades traumáticas más frecuentes por la que acuden enfermos a los servicios de urgencia de Traumatología. Aunque su tratamiento es por lo general conservador, existen situaciones en que es necesaria la intervención quirúrgica con la fijación externa. Objetivo: evaluar los resultados de la fijación externa en pacientes con fracturas inestables del radio distal. Materiales y métodos: se realizó un estudio preexperimental en pacientes con fracturas inestables del radio distal, tratados mediante fijación externa en el Hospital Provincial Docente Clínico Quirúrgico Manuel Ascunce Domenech, de Camagüey, entre abril de 2018 y mayo de 2021. La investigación tuvo un nivel de evidencia IV, recomendación D. Resultados: el promedio de edad de los 41 pacientes fue de 40,5 años, con edad mínima de 20 y máxima de 68. Se observó un predominio del sexo femenino (25 pacientes, para un 61 %). La razón sexo femenino-masculino fue de 1,5 a 1, y la de afección de muñeca izquierda-derecha fue de 1,9 a 1. Se encontró significación entre antes y después al emplear los parámetros imagenológicos y las escalas evaluativas, con mejoría en ambos. Conclusiones: la fijación externa en fractura inestable del radio distal resulta eficaz a los doce meses en la mayoría de los enfermos. Las complicaciones son mínimas, y de presentarse responden de forma favorable al tratamiento.


ABSTRACT Introduction: distal radius fracture is one of the most frequent traumatic entities due which patients come to Traumatology emergency services. Although their treatment is usually conservative, there are situations where surgery with external fixation is necessary. Objetive: to evaluate the results of external fixation in patients with unstable distal radius fractures. Materials and methods: a pre-experimental study was carried out in patients with unstable fractures of the distal radius treated by external fixation at the Provincial Clinical Surgical Teaching Hospital Manuel Ascunce Domenech, of Camagüey, between April 2018 and May 2021. The research has IV level of evidence, D recommendation. Results: the average age of the 41 patients was 40.5 years old, with minimum age of 20 and maximum age of 68 years. Female sex prevalence was observed (25 patients, 61%). The female-male sex ratio was 1.5 to 1, and the left-right wrist condition was 1.9 to 1. Significance was found between before and after using the imaging parameters and the evaluative scales, with improvement in both cases. Conclusions: external fixation in unstable distal radius fracture is effective at twelve months in most patients. Complications are minimal and if they are present, they favorably answer to treatment.

Presentamos un estudio epidemiológico de FEDR y lo comparamos con otras áreas de la península y Europa. Método: Se recogieron todas las FEDR de nuestra área sanitaria registradas en 2017 y 2018 en pacientes mayores de 18años de forma retrospectiva y observacional agrupándolos según la clasificación de AO-OTA. Se analizaron las variables de edad, sexo, época del año, mecanismo de fractura, presencia de lesiones asociadas y tratamiento realizado. Resultados: Registramos 1.121 FEDR en 1.108 pacientes, 903 mujeres (81,5%) y 205 varones (18,5%). En 612 casos se vio afectado el lado izquierdo (54,6%) y en 509 el lado derecho (45,4%). La edad media de la muestra fue de 65,9años (IC95%: 65-66,9años). La tasa de incidencia de FEDR fue de 158,5 fracturas por 100.000 habitantes/año. El 49,2% se clasificaron como tipo A de AO-OTA y se intervinieron quirúrgicamente un 19%. Conclusión: La incidencia de FEDR alcanza un máximo en la sexta década tanto en hombres como en mujeres. Las fracturas de tipo A de la clasificación AO-OTA son las más frecuentes. La mayoría se trataron de forma conservadora. El patrón de paciente tratado quirúrgicamente con más frecuencia es el de una mujer mayor de 50años con FEDR articular compleja.(AU)

Introducción: Cerca de 200 millones de personas en el mundo sufren de osteoporosis. En promedio ocurre una fractura cada 3 segundos y su prevalencia ha ido en aumento de manera exponencial en la última década. A pesar de ser un problema de salud pública en Colombia carecemos de estadísticas que muestren la magnitud de la enfermedad. Materiales y métodos: Estudio descriptivo transversal con componente analítico de pacientes con fracturas por fragilidad atendidos por el servicio de Urgencias. Se calculó su incidencia, caracterización de la población y asociaciones con significancia estadística entre las variables de interés. Resultados: Se encontró una incidencia de 10.7 FF /100 fracturas/año y 1.02 FF/100 pacientes atendidos en el hospital/año. Mayor incidencia a los 57 años, 80% en mujeres, principalmente cesantes. La fractura de radio distal tuvo mayor prevalencia seguida de cadera y húmero proximal. El 85% de los pacientes no tenía antecedente de osteoporosis. Se encontraron asociaciones estadísticamente significativas entre edad y localización, sexo y edad, sexo y localización como principales. Discusión: Con el envejecimiento poblacional, la incidencia de FF ha ido en aumento, encontramos resultados demográficos similares a los reportados en la literatura, una mayor frecuencia en mujeres y comprometiendo principalmente el radio distal. Debido a las limitaciones metodológicas del estudio éste nos sirve como generador de hipótesis basados en las asociaciones encontradas. Consideramos importante la identificación, diagnóstico e inicio de tratamiento en estos pacientes pues se disminuye el riesgo de nueva caída, morbilidad y mortalidad asociadas. Nivel Evidencia III: Estudio Descriptivo - Analítico.


Introduction: Nearly 200 million people in the world suffer from osteoporosis. On average, a fracture occurs every 3 seconds and its prevalence has been increasing exponentially in the last decade. Despite being a public health problem in Colombia, we lack statistics that show the magnitude of the disease. Materials and methods: Cross-sectional descriptive study with an analytical component of patients with fragility fractures treated by the Emergency Department. Its incidence, characterization of the population and associations with statistical significance between the variables of interest were calculated. Results: An incidence of 10.7 FF/100 fractures/year and 1.02 FF/100 patients treated in the hospital/year was found. Higher incidence at 57 years of age, 80% in women, mainly unemployed. Distal radius fracture had the highest prevalence followed by hip and proximal humerus. 85% of patients had no history of osteoporosis. Statistically significant associations were found between age and location, sex and age, sex and location as the main ones. Discussion: With population aging, the incidence of FF has been increasing, we found demographic results similar to those reported in the literature, a higher frequency in women and mainly involving the distal radius. Due to the methodological limitations of the study, it serves as a generator of hypotheses based on the associations found. We consider the identification, diagnosis and initiation of treatment in these patients to be important as it reduces the risk of new falls, associated morbidity and mortality. Las fracturas del radio distal están entre las patologías más comunes en el área de la emergencia, actualmente los tratamientos más utilizados son la fijación con alambres de Kirschner y fijador externo (AKFE), o la osteosíntesis con placas volares de estabilidad angular (PVEA), realizamos un estudio comparativo de los resultados a corto plazo entre ambos métodos. Se realizó un estudio transversal, retrospectivo y descriptivo desde el 2005 al 2019, donde se revisaron las historias de los pacientes con fracturas de radio distal tratados con AKFE o PVEA, evaluando el alta después de la medición de la amplitud de movimiento articular funcional. Se incluyeron 68 pacientes, 47(69,11%) pacientes tratados con PVEA y 21(30,88%) con AKFE, la edad promedio fue de 53,07 años, la causa más común fue la caída de su altura en 60,29% casos, el alta médica por mejoría se realizó a las 15,95 semanas para AKFE y a las 9,86 semanas para PVEA (<0.05). La osteosíntesis con PVEA evidenció un menor tiempo de recuperación de la amplitud de movimiento articular con respecto a los pacientes tratados con AKFE(AU)


Fractures of the distal radius are among the most common pathologies in the emergency area, currently the treatment more frequently used are fixation with Kirschner wires and external fixator (KWEF), or osteosynthesis with angular stability volar plates (FAVP), we carried out a comparative study of the short-term results between both methods. This is a cross-sectional, retrospective and descriptive study from 2005 to 2019, where the histories of patients with distal radius fractures treated with KWEF or FAVP revised, evaluated, evaluating at discharge after functional joint range of motion was measured.68 patients were evaluated, 47 patients treated with FAVP and 21 with KWEF, average age of 53.07 years, the most common cause was the fall from his height with 60.29%, the medical discharge was made at 15,95 weeks for KWEF and 9,86 weeks for FAVP; with a t student<0.05 between them. Osteosynthesis with FAVP evidenced a shorter recovery time for joint range of motion compared to patients treated with KWEF(AU)

ANTECEDENTES: Las fracturas de radio distal Fernández III (FRDF III) son una urgencia muy frecuente. Las indicaciones quirúrgicas y no quirúrgicas persisten controversiales, así como el impacto funcional. OBJETIVO: Comparar la evolución clínica de los trabajadores con FRDF III, con manejo quirúrgico frente a no quirúrgico, no complicados, en un hospital especializado. MÉTODO: Estudio comparativo, observacional y longitudinal en trabajadores con FRDF III unilateral cerrada, de ambos sexos, con evolución menor de 5 días. Se excluyeron pacientes con comorbilidad afectante de consolidación ósea y polifracturados. Se eliminaron aquellos con información incompleta, con complicaciones, con aparato de yeso modificado 5 días después de su colocación, con mala reducción-estabilización acorde a la AO. Se aplicó la Escala Funcional de Mayo para Muñeca (EFMM) a los 3 y 6 meses del tratamiento. Se utilizó la prueba t de Student. RESULTADOS: 159 pacientes, 34% mujeres, 66% hombres. Edad promedio 40.18 años. Manejo quirúrgico 42,5% (placa 8.7%, fijador 29%, fijador + clavillo 8.7%); no quirúrgico 33%. Diferencias significativas (p ≤ 0.05): EFMM 3 y 6 meses; a 3 meses: dolor, prensión; a 6 meses: movilidad; y días totales de incapacidad. CONCLUSIONES: El tratamiento quirúrgico muestra mejores resultados funcionales, destacando el dolor y la prensión a 3 meses, y la movilidad a 6 meses, con reincorporación laboral en menos tiempo. BACKGROUND: This fracture is a very frequent emergency. Surgical and non-surgical management are still controversial, such as functional impact. OBJECTIVE: To compare clinical evolution of Fernandez III distal radius fractures, managed with surgically vs no surgical treatment, in a specialized hospital. METHOD: Comparative, observational, longitudinal study, released on workers with unilateral Fernandez III closed distal radius fractures, both sex, with evolution less than 5 days. Those patients with pathologies getting difficult in bone consolidation, and with several fractures were excluded. Those with incomplete information, complicated, and whom paste were modified after 5 days, and with bad reduction or stabilization fracture according AO were eliminated. Mayo Functional Wrist Scale (EFMM) was applied at 3 and 6 months. Student-t test was applied. RESULTS: 159 patients recruited, 34% females, 66% males. Medium age 40 years old. Surgical treatment 42.5% (plaque 8.7%, external fixation 29%, external fixation and nails 8.7%). Non-surgical treatment 33%. Significative differences (p≤0.05): EFMM at 3 and 6 months; pain, grasp at 3 months; mobility at 6 months; total laboral inhability days. CONCLUSIONS: Surgical treatment shows better functional results, specially pain and grasp at 3 months, mobility at 6 months, and faster returning to work.

Abstract Objective The present study aimed at analyzing the clinical, radiological and functional results of the reconstruction of the distal radius after tumor resection with a custom-made metal arthrodesis implant and compare them with other types of distal radius reconstruction, as presented in the literature. To our best knowledge, this is the first article describing this particular type of implant and patient functionality. Methods Functional outcomes of reconstruction of the distal radius were assessed in a series of 4 patients. Three of the patients having had resection of giant cell tumors (GCTs), one patient having had resection of osteosarcoma. Results There were no major implant-related complications like infection, nonunion or loosening. Two patients had to undergo further surgery for protruding metalwork. Overall function was good according to the Musculoskeletal Tumor Society MSTS and Disabilities of the Arm, Shoulder, and Hand (DASH) scores. Conclusion The present study shows that custom-made metal arthrodesis implant benefits from the fact that it can be used as a salvage option when other treatments have failed, or it can be used as a primary option in cases in which there is limited bone stock after distal radius tumor resection.


Resumo Objetivo O presente estudo teve como objetivo analisar os resultados clínicos, radiológicos e funcionais da reconstrução do rádio distal após a ressecção do tumor com implante metálico personalizado de artrodese e compará-los com outros tipos de reconstrução do rádio distal, conforme apresentado na literatura. Pelo que conhecemos, este é o primeiro artigo descrevendo esse tipo particular de implante e funcionalidade no paciente. Métodos Os desfechos funcionais de reconstrução do rádio distal foram avaliados em uma série de 4 pacientes. Três dos pacientes tiveram ressecção de tumores de células gigantes (TCGs), sendo um paciente com ressecção de osteossarcoma. Resultados Não houve complicações relacionadas ao implante, como infecção, não sindicalidade ou afrouxamento. Dois pacientes tiveram que passar por uma nova cirurgia para a protusão da prótese metálica. A função geral foi boa de acordo com as pontuações da Musculoskeletal Tumor Society (MSTS) e Disabilities of the Arm, Shoulder, and Hand (DASH). Conclusão O estudo mostra que o implante metálico personalizado de artrodese se beneficia do fato de que pode ser usado como opção de salvamento quando outros tratamentos falharam, ou pode ser usado como opção primária nos casos em que há estoque ósseo limitado após a ressecção do tumor do rádio distal.

RESUMO

Objetivo: Proponer la centralización del cúbito distal en los grandes defectos óseos secundarios a pseudoartrosis u osteomielitis tras fracturas abiertas de radio distal, como técnica de rescate cuando no estén indicado los injertos libres vascularizados. Material y métodos: Revisamos los casos de dos pacientes con destrucción del radio distal como complicación séptica de una fractura abierta a ese nivel, que fueron tratados mediante resección del hueso afecto y espaciador de cemento con antibiótico como tratamiento inicial para, en un segundo tiempo, una vez controlada la infección, realizar una centralización del cúbito y artrodesis cubitocarpiana. Resultados: Ambos casos precisaron una cirugía adicional debido a pseudoartrosis, uno a nivel de la osteotomía y el otro a nivel de la artrodesis cubitocarpiana. A los 10 años ambos casos presentan control de la infección y ausencia de dolor. Conclusiones: En nuestra experiencia, la técnica de resección amplia de la pseudoartrosis séptica del radio, centralización del cúbito como hueso único y artrodesis cubitocarpiana es una opción válida como técnica de rescate en las infecciones de radio distal con una extensa pérdida ósea.(AU)


Objective: Propose the centralization of the ulna technique as a therapeutic alternative in large bone defects secondary to non-union or osteomyelitis after open distal radius fractures as a salvage technique when vascularized free graft are not indicated. Material and methods: We reviewed the cases of two patients with distal radius destruction as a septic complication after an open fracture who were treated by resection of the affected bone and a cement spacer with antibiotic as initial treatment. After the infection was controlled, perform a centralization of the ulna and ulnocarpal arthrodesis. Results: Both cases required additional surgery due to non-union, one at the osteotomy level and, the other one, at the ulnocarpal arthrodesis level. After ten years, both cases have infection controlled and no pain. Conclusions: In our experience, the technique of broad resection of septic radius non-union, centralization of the ulna as a single bone and ulnocarpal arthrodesis is a good option as salvage technique in distal radius infections with extensive bone loss.(AU)
